Instructions for obtaining an IFSS Driver Identification (DID) Number

1. Download and complete the DID form from the USFSS website – usfss.org - or simply provide your name, date of birth and gender.

2. Email the form (or the above necessary information) to the USFSS Treasurer at:

3. Send a check for $56.55 made out to USFSS to the address above or pay your $56.55 via the PayPal link below.

NOTE: You must be a USFSS member to request a DID. Membership information can be found here:

Please allow a week to 10 days for the processing and issuing of the DID.
